Hybrid Students Medication Reminder

Our highest priority at MHHS is the health and safety of our students as we prepare for their return in January 2021.

If your student will be attending campus classes and has a medical condition that requires medication, please be sure to complete the required Medication at School form prior to our return.

Any medication(s) can be kept in the Health Office or with the student, please sure to designate as such at the bottom of the form. If the medication(s) are to be kept in the Health Office, please provide the medication(s) along with the completed Medication at School form prior to our return.
